  •  Our Program Goals:
  •       Assist clients with little or no income.
  •      Assist and refer clients to provide increased access for children and adults for eye glasses and reconditioned hearing aids. 
  • Our Guidelines:
  •      Clients may qualify if:
  • The client meets the income criteria.  Low-income families have no available assets that can be quickly liquidated. 
  •  The client is not eligible for Medicaid.
  •  A client experiencing a temporary income hardship (i.e. loss of employment and/or loss of any type of income but not limited to public assistance, cash grants, SSD, SSI and Food Stamp assistance).
  •   Clients will not qualify if:
  •  They receive Medicaid
  • They accesse GFL before the required two-year wait time.
  • Our Procedure and/or Process:
  •  Not mandated but Family Service prefers, if clients are referred by health providers, teachers, school nurses and social workers.
  • Service is provided by appointment only.
  • A face-to-face interview must be completed.
  • Client must have documentation.  Social Security Card and/or income verification.  The Executive Director and/or the Executive Assistant have the right to ask for these requirements and the client must comply.
  • The Family Service Association reserves the right to refer a client to a State Program that may assist in providing Medicaid.  Also refer to other health providers that contract with the Glens Falls Lions Club.
