The order form is attached. Paper copies of the brochure and order form will be coming home this week. The information will also be posted on our website: http://mysite.verizon.net/aharvey/cvillepmc/
Please return your order form and check (made out to CPMC) to the CPMC mailbox in the band room. No cash will be accepted. Order due date has been extended to November 20th. Delivery will be Friday December 4 from 5:00- 7:00 pm on the stadium side of CASH.
