Forrestville Valley Area Book of Military Personnel
The Forreston Illinois American Legion Post 308 invites all military personnel and veterans from the
Adeline, Baileyville, Brookville, Florence, Forreston, German Valley, Haldane, Harper, and Leaf River area to submit their
name for inclusion in their "Roll of Honor" book of "Hometown Heroes".
Names submitted will have lived in the area (roughly encompassing the Forrestville Valley School District)
sometime during their lifetime. Names may also be submitted by a family member.
Civil War era to the present will be included. Please include your hometown, era, and branch of service such
as Air Force, Army, Coast Guard, Marines, National Guard, Navy, Reserves, WAC, etc.
Photos in uniform will also be accepted. Other information requested includes occupation at time of enlistment
or draft, married or single, any service awards, dates of service, highest rank, Battalion, Regiment, Division, Unit, Ship.
If the family member is no longer living, you may include birth and death date and cemetery (plus county and
state). Memories, stories, and thoughts may also be submitted. Forms are currently available at Forreston Library, Forreston
State Bank, Forreston Village Hall, Forreston Hardware, German Valley State Bank, and Leaf River Library.
Completed forms may be dropped off at Forreston Village Hall (water bill slot 24 hour access) or information may
be mailed to: George Martens, 9465 N. Mt. Vernon Rd., Forreston, IL 61030. For more information you may call George Martens

Photos and information may also be submitted online to paschpark@msn.com. If you have any problems printing the form below, please email us and we will email it to you. Please submit information
by April 30, 2008. All names currently on the Veteran’s Memorial in Forreston will be automatically included. Please
use this form for additional information.
Books will be available for purchase after completion.
Photo submission sessions will be held prior to the April 30, 2008 deadline at Forreston Public Library.
Photos will be scanned while you wait and returned to you immediately.
A photo submission session will be held Monday, March 10, 2008 from 6-8 at the Forreston Public Library.
