coverage call the phone number on your card then ask and document the following:
- Name
of the person you spoke with.
- Does your plan cover nutrition counseling or medical nutrition therapy
(codes 97802 & 97803).
If
you are covered, then continue to ask and document the following:
- Must you see a certain provider? (If so who?)
- Will your insurance pay a certain percentage?
- Is it out of network or in-network?
- Do they actually contract with any dietitians for in-network
coverage?
- Is there a limit on the "usual and customary fee"?
- Do you need a doctor's referral?
