The following list includes insurance companies we partipate with as of 9/11/2009.  We encourage you to contact your insurer directly to confirm our current participation with your health plan.

Workers Compensation cases are accepted only with prior payment authorization from the workers compensation insurance carrier. 
 
Automobile accident cases are accepted only with PIP (Personal Injury Protection) or MedPay from patient's own automobile policy.  We do not bill third parties, i.e. the person who caused the accident.  We do not bill attorney's.  We do not accept payment arrangements from attorney's or other insurers.   Patient who receives the services in our office is fully responsible for payment.

Many insurance companies require referrals, prescriptions, or authorizations for physical therapy.  You are responsible for verifying your benefit requirements before visiting our office.  Obtaining the necessary referral from your primary care physician or prescription from the physician ordering physical therapy is the patient's responsibility.  Our staff will be happy to assist you in obtaining authorizations for services if necessary.
