Patient Rights and Responsibilities:
- You must have an appointment. Please be aware this not a "walk-in" clinic.
- Keep your scheduled appointment or notify the clinic in a timely manner if you must cancel.
- Bring your medicine/prescription bottles with you to each clinic visit.
- Be aware that the clinic doctors do not prescribe controlled substances or narcotics.
- Be courteous to other patients, volunteers, and staff.
- Do not drop in without a scheduled appointment.
- You have a right to confidential, respectful, non-discriminatory and non-judgemental care.
- You have a right to consider treatment options. You may refuse treatment.
- If you do not meet our financial guidelines, we will provide you with alternative approaches to obtaining health
care.

ODC believe:
