This form will allow you to submit a family for the Tinder database.  Enter the information in each field and then push the submit button at the bottom of the form.  The information will be e-mailed to me.  If the couple has more than five children, submit the first page then clear the form.  Add the husband's name back on the form and then continue adding children.  Push the submit button to send the additional information.  Continue doing this until all information has been sent.  If you know that the children have been married, and have children of their own, do a separate Family Group Sheet for each child's family.  Please put your first name in the "Submitters Name" field and your e-mail address in the "Submitters e-mail Address" field so that I can contact you with any questions.  Thank you in advance for your information.

WARNING:  When typing in the text fields DO NOT hit the "ENTER KEY" on your keyboard.  This will cause the form information to be mailed at that point.  You will then have to start over, and I will get an incomplete form submitted.  When you have finished typing information in a text field hit the "TAB KEY" to move to the next text box.  When you have all the information in the form that you wish to submit, press the submit key at the bottom of the form.  I have a question entered to the software manufacturer to see if there is a way around this problem.  Thanks!
