Print this form out, complete it and mail to Thunder on the Avenue 2550 Franklin Drive Vineland,
NJ 08361 before September 13, 2006 along with a check for $15 (rider) or $10 (passenger).
By signing this form, I release the Thunder on the Avenue Committee, City of Vineland, VDID/Main
Street Association and all of its members from any and all liabilty issues that may arise from participating in this event.
All pre-registration forms Must be received on or before September 13, 2006.
