Application checklist
Your application should be accompanied by these items:
- A completed application form with all required information
- Proof of age. A copy of your driver's license, birth certificate, or passport is acceptable
- Two copies of your program. For each work give the title, composer, composer's birth and death dates, and accurate timing
- A check or money order for the appropriate application fee
