I've only made two changes to the stock exhaust; a FlowMaster 40 Series Muffler and a Borla Intercooled Exhaust Tip.  I would have liked a nice cat-back system from Gibson or MagnaFlow or someone, but I could never find one that the manufacturer would confirm would fit my truck.  They always refer to a King Cab Short Bed and they couldn't confirm that their system would fit my '04 King Cab 74" bed.   Oh well.
 
The exhaust tip looks nice and I'm happy with the sound of the FlowMaster.  It's got a bit of a hum while accelerating, but at speed it's really pretty quiet.  I don't like a very quiet exhaust; I like hearing the vehicle running (especially with a manual transmission).

UPDATE: After a couple of months driving in the cooler weather with all of the windows closed; I have to say that the exhaust is not really noticeable at speed (typically 2500 to 3000 RPM).  There's more noise from the engine compartment than from the exhaust.

FWIW, I recorded my exhaust with my Nikon 5600.  I think it's actually a pretty accurate recording.  I wish I had the camera before I had the muffler replaced to do a "before" clip.  Any difference in sound is due strictly to the FlowMaster.  I had the Borla exhaust tip on there before the muffler and it did not change the exhaust note/sound at all.

The camera is about 3 feet from the exhaust and I revved it to about 3500 RPM.
