Thank you for providing information related to the request for death
benefits
under this policy. I would like to process the death benefits claim as
quickly as possible. However, some additional information is still
needed.
Please allow me to explain.
I want to begin by confirming that the policy names Samuel and Emma
Peltzer
as primary beneficiary. However, because Samuel and Emma Peltzer died
before
Mary Peltzer, the death benefit is now payable to the estate of Mary
Peltzer.
Once I receive the following material, I can begin to process the claim.
Please send these documents to us in the enclosed reply envelope.

If
the
last will and
testament of Mary Peltzer is being probated, please
submit a copy of the court order appointing the executor of the estate.
If
the insured did not leave a will, and an administrator is appointed,
please submit a copy of the court order appointing the administrator of
the estate.

If there is no
executor
or administrator appointed, based on the total
policy proceeds of $4,111.00, these general guidelines may be followed:

|
— If payment will be made to a creditor or a person
who has paid the
funeral expenses, please complete and return to us the enclosed
Small
Estate Affidavit for New York labeled A. Note that there is a
waiting period of 180 days after the death of the decedent
before the affidavit can be completed and returned. It is
important
that you do not sign and date the affidavit until after the
applicable
waiting period has expired, or a new affidavit will need to be
completed.
— If payment will be made to
adult children, parents, siblings, nieces
nephews, or a creditor, please complete and return to us the
enclosed
Small Estate Affidavit for New York labeled B. Note there is a
waiting period of 30 days after the death of the decedent
before the affidavit can be completed and returned. It is
important
that you do not sign and date the affidavit until after the
applicable
waiting period has expired, or a new affidavit will need to be
completed.
— If payment will be made to a spouse, please
complete and return to us
the enclosed Small Estate Affidavit for New York labeled C.
— If none of the above guidelines apply, the state of
New York requires
that the last will and testament of Mary Peltzer be probated and
an
executor appointed. Please submit a copy of the court order appointing
the executor of the estate. If there is no will, an administrator must
be appointed. Please submit a copy of the court order appointing the
administrator of the estate.

If there are other heirs to the estate, please list each in the space
provided on the enclosed affidavit. If the heirs wish to release their
share of the benefits, each must complete a copy of the enclosed
release form (Comb 9330). If they wish to claim the benefits, each must
sign the
affidavit and complete a copy of the enclosed claim form.

The enclosed Life
Insurance Death Benefits Claim Form should be completed by the
appointed executor or administrator of the estate (if there are
co-executors / administrators appointed, each should sign the claim
form), or the person completing the affidavit. Be sure that Section 6
titled Tax Certification is completed and that Section 7 titled
Signature is signed.
Please send the insurance policy if available.
Please send us the original death certificate issued by the state where
Mary Peltzer passed away, or a copy that has been certified and
includes the raised seal of that state. To prevent a delay in handling,
the death certificate should include the cause or manner of death.
Because the death certificate becomes part of the claim file, it cannot
be returned to you.
