This is a therapeutic practice. The session will be cancelled or terminated without refund if the client is under the influence of alcohol or recreational drugs upon arrival, or behaves in an inappropriate or sexual manner.

New Clients:

Ø    Complete a health care intake form prior to first appointment

Ø      Allow a few additional minutes for the first visit to review the health history and intake forms, and do an initial assessment.

All health and personal information is kept strictly confidential.

Late arrivals will pay the full fee, but the services will be offered only for the remaining time of the scheduled appointment.

No insurance billing is done by this office. If you have insurance coverage for reimbursement of fees, appropriate documentation may be provided so that you as the client may seek reimbursement.

Referrals are welcome from other health care professionals.
