We now have a new hotel rate of $149 per night. Please use following updated Registration Form 4/14/09 and RSVP by May 18th.

The downloaded form is a PDF fill-in form.

Options to submit are:

1. Print form, hand fill in needed information and mail back.

2. Fill-in the form online, save it to a file, email back to nyyft@verizon.net the file as an attachment.

Instructions for online submission:

Go to http://www.win2pdf.com to download Win2PDF software.

1. Install the Win2PDF software on your computer and it will load as a new printer called Win2PDF.

2. Then download the PDF registration form. It will open with Acrobat Reader, fill-in the needed information.

3. Select Print from the file menu and choose the Win2PDF printer.

4. From the Win2PDF File Save dialog, choose a file name and save or e-mail the PDF file.
