APPT-HOURS-PAY

Hours: Monday through Friday, between 7:30 am with the last appointment starting at 6:00 pm

Appointments: Call 804-677-1005.
		Please leave a message as I do not answer the phone while I am in with a patient.
		All treatments are by prior appointment only.
		If you call AFTER hours I will return your call during business hours the next business day. Calls on Friday evening or on the weekend will be returned during business hours on Monday.
		If you are having a Medical Emergency call 911 or go to the nearest emergency room!
		To help me respect your and other patient’s time it is important that you arrive on time!
		Cancellations: A scheduled appointment must be cancelled 48 hours in advance, or you will be billed for the appointment. Please cancel by phoning (804) 677-1005.

Payment:
		Payment is due at the time of treatment.
		Payment may be made in the form of Check or Cash.
		There is a $50.00 charge on returned checks. I haven’t had this happen yet, so keep up the good work!!!

Insurance: I am glad to help you fill out insurance forms so that you may submit them to your insurance carrier, but I do not accept insurance. I would encourage you to call your insurance carrier to check and see if acupuncture is covered. If so, check to see if you need your primary care physician’s referral and diagnostic codes.
