1. GROSS SALES (Total Revenue)                  List Total estimated sale for each month
2. Less: Returns & Allowances                   Goods returned or credit given to customers

3. Net Sales (1-2)                              Self-explanatory

COST OF GOODS SOLD
4. Beginning Inventory                          Inventory on hand as beginning of month
5. Plus:  Purchases                             Merchandise for resale or for use in product

6. Total Goods Available (4+5)                  Self-explanatory
7. Less:  Ending Inventory                      Actual inventory on hand at end of month

8. Total Cost of Goods Sold (6-7)               Self-explanatory
9. Gross Profit (3-8)                           Self-explanatory

OPERATING EXPENSES
10. Advertising                                 This ammount should be adequate to maintain sales volume-include telephone book yellow page cost
11. Automobile                                  If personal car is used, charge in this column-include parking
12. Depreciation                                Allowable by IRS. See Your Accountant
13. Dues/Subscriptions/Licenses                 Self-explanatory
14. Insurance                                   Coverages on business property and products e.g. fire, liability; also workman's compensation, fidelity, etc.
15. Interest                                    Remember to add interest on loan as it is injected
16. Legal and Professional                      Outside services, including, for example, bookkeeping, accounting
17. Office Supplies                             Items purchased for use in the business (not for resale)
18. Payroll Expenses (include taxes & benefits) Include paid vactions, paid sick leave, health insurance, unemployment insurance, etc.
19. Rent                                        Real estate only
20. Salaries                                    Pay to Management and executives
21. Taxes-Sales, etc.                           Inventory tax-sales, tax-excise tax, if applicable
22. Telephone                                   Self-explanatory
23. Travel & Entertainment                      Self-explanatory
24. Utilities                                   Water, heat, light, and/or power
25. Wages - Employees                           Base pay plus overtime (if any)
