Ceremony Booking Agreement
This document is a letter of agreement between the wedding couple whose signature appears
at the end of this letter and Ellen Orndorff/Raechal Martin. The date, place
and time of the wedding being booked is:

The following applies to this agreement:
1.
This agreement reserves officiating services at the place, date and time listed above to perform a ceremony.
2.
This letter
of agreement is contingent upon receiving a COMPLETE Information form. Any changes
in details require a new form to be submitted. Keeping this form up-to-date ensures.
That all scheduling is met.
3.
Any unpaid
balance of fees for officiating services will be paid at least two (2) weeks prior to the ceremony date.
4.
Officiant
is not liable to any costs, damages, or actions as a result of this agreement in excess of the amount paid for the officiating
service.
5.
In case
of emergency or any other unavoidable or resolvable incident, Ellen and Raechal reserves the right to substitute another Officiant
for the event time and day listed above without penalty or change in agreement. This
is to ensure that no matter what happens, you have an Officiant to complete your wedding as planned.
6.
A fee of
$50 for any bounced check will be paid at least two (2) weeks prior to the ceremony date.
7.
Deposits
are non-refundable. Any additional fee paid in advance is refundable up to 30
days prior to the Ceremony date with exception to bounced check fees. Refunds
are processed within 30 days.
