A send off report MUST be filed within 24 hours, we recommend within 4-6 hours, for each instance of a red card send off of a player. You must use one of the seven send off offenses and describe the action. For instance, if a player was dismissed for Abusive, Offensive, or Insulting Language and/or Gestures, be sure to list what the abusive... language was.

Also use this report to instances where other team personnel (coaches) are dismissed from a match. In the match report the referee would say that he dismissed or expelled the coach for his failure to conduct himself in a responsible manner. Then the referee would like the irresponsible act.
