The following forms from the National are for use in planning, promoting and reporting on your JAKES Field Day function. These forms may be downloaded by right-clicking and selecting Save, or can be viewed and printed directly from the web.

Please note that any New York State SuperFund request for funds for a local chapter JAKES Field Day (or Women In The Outdoors) event will automatically have $ 150 deducted from the requested amount to cover the cost of insurance for the event. This will be handled entirely by the National, and allows the use of firearms, airguns, and/or bows and arrows at these events.
