Subscription Information Snow Shoe EMS Service sends out subscriptions to residents every year in June. These subscriptions cover residents for the difference of what their insurance pays for a emergency ambulance call. If you do not receive one in the mail, please call 814-387-4499 and leave your name and address, we will make sure you get one in the mail. If you would like, we put extra blank copies at Halls IGA, you can pick some up there, please fill them out completely and either mail them to us, or give it the desk clerk for us to pick up. Subscription costs: Single (anyone who is over 18 and lives alone or at home except college students they are covered under the family subscription) $20.00 Family (includes anyone who lives in the same household and have the same address and is a dependant of the head of the household) $35.00 Subscription cost can change prior to the mailing. THANKS Paramedic Services Any billing from the paramedic service will be sent to your insurance, you will be responsible for the balance owed or the whole payment depending on what your insurance covers. This service is a separate charge from the Snow Shoe Ambulance charge. Routines We will do routine transports if we have an available crew and/or 24 hours notice. The physician must sign a Patient Necessity Form prior to the transport , any additional mileage or services that are not covered by insurance will be billed to the patient for payment. We do 3rd party billing. Which means that we send the bills from the emergency transport to your insurance company, if you are subscriber we accept what your insurance pays. If you are not subscriber we will expect payment for the balance due.
