The NALC is addressing the change in the FMLA form and will distribute an updated guide shortly.  It will be posted on this page when it becomes available.   The forms and instructions listed below are provided by the Department of Labor and MUST be used after January 16, 2009 to apply for protection under the Family Medical Leave Act.
