If your insurance provider is not listed on this page we will try to make an effort to submit on your behalf, applicable copayments are due at the time the services are rendered. If your insurance carrier is not on our list we can submit the bill on your behalf. You will be responsible for payment if the claim is denied for any reason.
 
We are providers of many different insurances. However, as in any medical procedure we cannot determine whether or not your insurance will cover the test. We will agree to accept assigment  and only bill you for that portion that is not covered ( IE co-insurance, deductibes and non covered).

Patients showing up without appropriate referral will not be seen. Please make sure you have a referral before you arrive.
