Medical Benefits

Beverage and Brewery Drivers Teamsters Local 67 Health and Welfare Fund

Deductible

There is a $200.00 per person calendar year Major Medical deductible with a per family maximum of $600.00. Not all of the benefits listed below are subject to this deductible.
 

Schedule of Medical Benefits

Pre-Admission Hospital

Tests Paid 70-30%

In-patient Hospital Room & Board (per stay)

100% of for first 90 days. Semi-Private Room

In-hospital services

100% of for first 90 days. Major Medical coverage after that.

In-hospital doctor visits
Normal

Paid in Full for 1st visit
Co-payment of $30.00 per additional visit

In-hospital doctor visits
Intensive Care

Paid in full for 1st visit
Co-payment of $30.00 per additional visit

In-hospital doctor visits
Consulting Physician

Paid in full for 1st visit
Co-payment of $30.00 per additional visit

Surgical Benefits

Reasonable and Customary charges in excess of the base allowance will be charged under Major Medical.

Assistant Surgeon

20% of allowed surgical charge

Organ Transplants

Paid in full up to lifetime maximum of $50,000. Advance Trustee approval required for transplants.

Hospice Care

If given less than 6 months to live. Hospice benefit is available. $5,000 for any one period.

Wellness

100% up to $200.00 annual benefit

Immunizations

80% of reasonable & customary up to $75.00. 

Alcohol & Drug Care

For mental, drug, & alcohol problems members must call Members Assistants Program (APS) 1-800-752-7242.

Out-patient

100% of 1st $1,000.00
Next $30,000.00 paid at 70%

Hearing Aids & Exams

100% up to $750.00 per

Prescription Drugs

Generic drugs with a co-payment of $7.00. Name brand drugs a co-payment of either $20.00 or $35.00.
