The Retail Pharmacy Program

For Your Short term and Immediate Prescription Drug Needs

At Participating Pharmacies

  1. Simply present your prescription drug ID card and prescription(s) to the pharmacist. Your eligibility for benefits will be confirmed via a computerized system.

  2. The pharmacist will tell you the correct co-payment (or coinsurance) you are responsible for paying.
You do not have to file a claim form for prescriptions filled at a participating pharmacy. To locate a participating pharmacy, visit the website: www.caremark.com.

If you use a non participating pharmacy

Caremark's local pharmacy program includes more than 60,000 participating pharmacies, including most major chains and more than 20,000 independent pharmacies around the country, to fill prescriptions for program participants. If you visit an in-network pharmacy without your Caremark prescription drug card, you may be required to pay 100 percent of the prescription price. You will then need to submit a paper claim form to Caremark for reimbursement of covered expenses. Be sure to fill your medication at a pharmacy in the Caremark network.

How do I submit a claim form?

1. Complete the Caremark Prescription Drug Claim Form.

2. Be sure to complete the participant and patient information.

3. Indicate the coordination of benefit information, if applicable.

4. Include the original prescription receipts (no copies).

5. Mail to the address indicated on the Prescription Drug Claim Form.
