Bypassing the rebate fulfillment center for the first time, I used Google to look up the address of the Customer Service Department of the manufacturer and sent the attached letter of complaint directly to them:

Dear Sir or Madam:
 

I filed for your $25 rebate in a timely and complete manner on July 15 of this year (documentation attached).  To date, I have gotten neither my rebate, nor any response to my multiple attempts to contact your rebate fulfillment center via letter, e-mail and telephone.

 

I would appreciate receiving your check for $25 no later than November 30, which would prevent further action on this matter.
