The Florida Adoption Reunion Registry -- "The department shall maintain a registry with the last known names and addresses of an adoptee and his or her parents whose consent was required under s. 63.062, and adoptive parents and any other identifying information that the adoptee, parents whose consent was required under s. 63.062, or adoptive parents desire to include in the registry."...."The registry shall be open with respect to all adoptions in the state, regardless of when they took place. The registry shall be available for those persons choosing to enter information therein, but no one shall be required to do so." Fla. Stat. ch. 63, section 165 (2001)
Fees -- "The department may charge a reasonable fee to any person seeking to enter, change, or use information in the registry. The department shall deposit such fees in a trust fund to be used by the department only for the efficient administration of this section." Fla. Stat. ch. 63, section 165 (2) (2001)
Proof of Identification -- "Anyone seeking to enter, change, or use information in the registry, or any agent of such person, shall present verification of his or her identity and, if applicable, his or her authority." Fla. Stat. ch. 63, section 165 (1) (2001)

Intermediary -- "The
court may, upon petition of an adult adoptee, for good cause shown, appoint an
intermediary or a licensed child-placing agency to contact a birth parent who
has not registered with the adoption registry pursuant to s. 63.165 and advise
them of the availability of same." Fla. Stat. ch. 63, section 162 (7)

with the Office of Vital Statistics in Jacksonville, Florida, stated in an email
to me the following: "An order appointing an intermediary must
come from the same court in which the adoption was entered. We do typically
suggest that court fees may be costly and the adult adoptee might first wish to
consider registering with the Florida Adoption Reunion Registry (F.A.R.R.) to
determine if a parent has already registered." -- more info about this

Counseling -- The department and agencies shall make counseling available for a fee to all persons seeking to use the registry, and the department shall inform all affected persons of the availability of such counseling. Fla. Stat. ch. 63, section 165 (2) (2001)
