Obtaining
Non-Identifying Information

at the Office of Vital Statistics stated in an email to me that they do not
provide information regarding date, place of finalization of an adoption, or the
name of an attorney handling an adoption to birth parent(s), so the following
information is for:
Adult Adoptees and Adoptive Parents Only
(Birth parents, see *Important*
below)
Non-Identifying information is, as the name
implies, information which does not include identifying information such
as names, addresses, dates of birth, etc.
Even if you are not actively
searching all adoptees and adoptive parents really should consider requesting
non-identifying information, if only to confirm that the information you have
corresponds with the state's records.
For
those
of you who are actively searching, non-identifying information may provide
additional information and clues you may not already have.
Upon request,
all non-identifying
information, including the family medical history and social
history, about the adoptee and the adoptee's birth parents "must" be
furnished to adult adoptees and/or adoptive
parents.

This information is
only available to adult adoptees and to adoptive parents.

Specific
names or identifying information are not included in the family medical or
social history.

Your
request should be made in writing.

Adoptees
should note in their request the names
of their adoptive parents.

A
copy of your driver's license or notarized proof of
identification is required, and you must state your role in this
adoption (adoptee, adoptive mother/father).

Important *
Birthparents,
even if you are not searching, you
really should update your non-identifying information with the Office of
Vital Statistics Adoption Unit. This way, even if your child is not yet
searching, valuable and important non-identifying information may be available
to them.

It is the belief of this site/registry owner that every person should have the
right to know their own medical history at the very least, and they deserve to
know about their own ethnic/social background as well.

you are searching, you should be very sure to register and keep your
information up to date with the Florida Adoption Reunion
Registry (FARR). Contact
the State of Florida Vital Statistics at 904-359-6990 for how you may confirm
that your child was adopted and when.

when available, must be furnished to the adoptive parents before the adoption
becomes final and to the adoptee, upon the adoptee's request, after he or she
