Appointments: Leave the date & time requested, and reason for the appointment (urgent sick
visit, preop for surgery, complete physical, complex visit (multiple problems).
Referrals: Leave the doctor's name, address, phone number, and reason for the visit. You must leave
your insurance information - the name of your insurance and your insurance identification number.
Questions to the doctor: You may ask questions related to a recent visit or lab test. We are unable
to make a diagnosis on any new problems, so please make an appointment for new problems. Also, if you are in the middle of
a complex situation, you may be asked to make another appointment. Please note which doctor you saw, when you were seen for
the problem, and briefly describe the problem & question.
Refills: We prefer you call your pharmacy for refills so that it can be done electronically - this is
not only more efficient, it also reduces errors.
