Coaches and other Volunteers:

 

The parents and children of the Nazareth Area Soccer Club(NASC) are very appreciative of your willingness to volunteer your time to help provide an effective soccer program for the Nazareth Area community.

 

In order to provide safeguards to protect the children in the program, and to comply with certain township requirements, NASC has instituted a background check program for all coaches, assistant coaches and other volunteers who have repetitive contact with the players.  Soccer Club Board members are also subject to this policy.  Please note that it is the intention of NASC to pay the fees involved in following this policy.  NASC must have all volunteers checked in order to qualify for use of fields and potential financial support from Lower Nazareth Township.

 

You must participate in the background checks to be able to coach.  As we start the program, volunteers are required to submit to the following:

 

1)       PA State Police Criminal Record Check

2)       PA Child Abuse History Clearance

 

We are working with Lower Nazareth Township to carry out this program, as well as to comply with certain requirements of the township.  The results of the checks will be provided to the township.

 

To process the background checks, we need to have the State Police checks submitted.  When these come back, they are attached to the Child Abuse Clearance forms for submission.  The following forms to be completed:

 

1)       Lower Nazareth Township Certified Sports Program Volunteer Application

      -include a copy of your drivers license

2)       Pennsylvania Child Abuse History Clearance

NASC will submit all State Police checks with payment.  With the complete State Police checks in hand, NASC will then submit the Pennsylvania Child Abuse History Clearance forms for processing.  The response from the state for the Child Abuse Clearance will be mailed direct to the home of the volunteer being checked.  You must then submit a copy of that response to the address above.

 

 

NOTE THAT YOU MUST SUBMIT ALL FORMS!!  In lieu of mailing the forms, you may fax everything to 484-214-0173.  This fax comes directly into my e-mail, so your information will not be floating around in some office.
