10 - 15% of all couples
 
PREDOMINANT AGE
Increases with age,
ages 30 - 34: 14% infertile
ages 35 - 39: 20% infertile
ages 40 - 45: 25% infertile
 
SIGNS AND SYMPTOMS
Thorough history and physical should be performed on each partner
 
Genital/pelvic infection (i.e., pelvic inflammatory disease) 
often associated with an obstruction of reproductive tract
 
Endocrine dysfunction (i.e., hypothyroidism, hypogonadism, abnormal puberty)
often associated with abnormalities of ovulation or sperm production
 
Sexual dysfunction (i.e., premature ejaculation) may contribute to the problem
 
Menstruation without egg production can occur without premenstrual symptoms.
Some patients may have facial/chest hair, obesity, acne which may be
suggestive of polycystic ovarian syndrome
 
Endometriosis is often associated with dysfunctions of menstruation.

Male factors 30 - 40%
 
Ovulation factors 15%
 
Cervical/ uterine factors 10%
 
Tubal/peritoneal factors 25 - 30 %
 
Immunologic factors 5 %
 
Psychogenic/nutritional/ metabolic factors 5%
 
Unexplained infertility 10 - 20 %

SEXUAL DYSFUNCTION IN WOMEN
 
DESCRIPTION
Difficulty getting or staying sexually aroused, reaching orgasm too quickly, difficulty or inability to reach orgasm, distaste or revulsion with sex, too little foreplay, too little tenderness after intercourse.

INCIDENCE/PREVALENCE IN USA

1 in 5 women are sexually dissatisfied, and two thirds of women report some degree of sexual dysfunction. Only one third of women who do not have orgasms in general think it is a problem.

Overall prevalence for dysfunction is 15 - 30% of all women. Desire disorders are complaint of 30 - 55% of individual patients presenting to clinics, and 31% of couples; arousal disorders present in about 14-48% in community studies. Orgasmic disorders probably the most common; about 10% primary, and up to 65 - 80% secondary in the community studies.

There are many barriers to seeking help, including embarrasment of expressing sexual inadequacy or fear of unknown therapy.

Can occur in post-pubertal age group; women's ability to experience orgasm increases gradually from puberty; in later teens nearly half have not had orgasm; by mid-thirties, about 10% have not

TREATMENT
