The insurance purchased by the Fundamental Baseball Camp Inc. for campers provides
coverage on an excess basis only. This means that the personal or group insurance owned by the parent or guardian
of the camper is the primary carrier and the camp insurance is the secondary carrier only, up to the limits and deductible
stated by the camp’s policy. If a camper becomes ill or in need of medical treatment while at camp, any payments due
to health care providers is the sole responsibility of the parent or guardian. The Fundamental Baseball Camp Inc., it’s
officers, directors and employees shall not be responsible for clothing or personal possessions lost or damaged by fire, theft,
laundry, malicious mischief or personal negligence. I hereby waive and release the Fundamental Baseball Camp Inc. from any
and all liability from any injuries or illnesses incurred going to camp, while at camp, or returning from camp to home. Said
waiver shall include all negligent conduct except gross negligence. No cancellation or refund of tuition will be granted unless
the camp is notified at least 8 weeks before camp opens. In that event, we will deduct $50.00 Administration and Handling
Fee, returning the remainder of the full tuition.

Every
student is advised to bring lunch in a small cooler. A concession stand may be available for snacks only. Each student must
bring his own bat and glove. We strongly advise that all items brought from home be labeled with a permanent marker. Additional
supervision may be available for those in need. Call for any information regarding directions, transportation, and rain schedules.
