Proclamations In Your Community

 

As an advocate for Childhood Stroke Awareness, you need to make sure your elected officials are educated on Childhood Stroke and legislation that impacts this important issue.  A great way to create awareness is to have a formal proclamation issued recognizing May 3, 2008 as Childhood Stroke Awareness Day.  You can ask that this be done at the local (i.e. city, town or county) level or at the state level.  The first step in this process is to complete the form below.

 

The Proclamation Project is not accepting any more requests at this time.  Check back often for updates to the project.

Completing the form below will assist us in tracking the success of our Proclamation Project.  Please be sure to complete each question in it's entirety.  Contact information is vital if follow up by the campaign or legislators is needed.

Guidelines for sending a proclamation request

 

CONTACT the office of your mayor/council chair/governor (either on your own or by letting the campaign send a request on your behalf) and explain that you are requesting a formal proclamation announcing May 3, 2008 as Childhood Stroke Awareness Day. You can see a sample letter above.  The following should be included with your letter and can be found on the Childhood Stroke Awareness Kit page:

ü      A copy of last year’s proclamation if one was received

FOLLOW UP with a polite phone call a few days after sending your request, asking a member of the office staff to confirm receipt of your request and to get an estimated time frame of the processing of the proclamation. Be sure to get the name of the person you speak with, thank them profusely, and ask if you may call them back to follow up. Continue to follow up, either by phone, letter or in person, but always be polite and thank the person you are dealing with for their efforts in helping survivors of Childhood stroke.

SEND a thank you note to your contact once you have the proclamation.

 

NOTIFY the Childhood Stroke Awareness Campaign Coordinator at childhoodstrokeawareness@live.com upon receipt of the proclamation so that we can officially thank your elected official for his/her efforts in promoting Childhood Stroke Awareness.  Notifying us at childhoodstrokeawareness@live.com will also insure that your proclamation will be listed on our website.
